Follow Instructions on the left of the Instructions Form to Sign Up

Once your sign-up is completed and approved, begin completing forms:

Under “Get Ready” – complete Contact Info, Insurance, General Info, Heart Health, Bone and Joint, and Medical Questions (you will see these again – answer them thoroughly and honestly).

• When you have completed all forms, the red flags will disappear
• When completed, click Done (blue top left)

Go to Things to do for “your name” at Northwest College – click Gets Started

• Find E-Signature Agreement – fill out electronically, fill in all blanks, by typing your name and date, you are approving that to be your legal signature. 
  • When completed, click Sign & Complete, marker will turn green; then you can move on.
• Complete all forms (14).
